How did he propose?

He proposed on my birthday. He told me to have my phone on and wait in instructions. I had the day off and he called me told me to get ready. I wore my favourite pink dress and he picked me up. He got me a birthday card with Goldeyes tickets to a game. He took my Tony Roma's and asked if I wanted wine which I had a glass. I looked at the dessert menu as I wanted to pick a one for later and he told me not worry as this was taken care of. Throughout dinner, he kept talking about our relationship. After we were done, dessert came out and I blew out the candle. He asked if I made a wish, I said no. He you should have as you never know if it might have come true. He continue talking about our relationship and pulled the ring and began to proposed. First as I was in shocked laughed and the I started to cry. He asked again and then I said yes. The restaurant knew ahead of time as he planned it. I kept noticing the kitchen staff kept looking and I couldn't figure until it happened.
