I don't have a picture, but I designed them up myself and printed them out on card stock and included the rehearsal dinner invites in the envelopes of the wedding invites for those who were invited.

Haven't thought of invitations...it's pretty casual so maybe just an e-mail. I have never received an invitation for the rehearsal.
