Marriage license application

So I’m trying to get the last of the last sorted out and I just started filling out the wedding license application.
some of the boxes are a little confusing; parents first and middle names. Do I NEED to put my parents middle names ?
as well as there is a box that says “intended place of marriage” and then the next one says “city” what would the intended place of marriage be if the city is going in the next box?

    Names for parents are needed as required and the city your getting married is what they are asking you. This is to certify that the officiant is putting all this information through for the government and your marriage license will have that city too along with your names.

    I'm such a worrier about little details like this! I would recommend reaching out to the courthouse/office and asking them to confirm anything you're unsure of. At least then you're 100% sure the form is filled out correctly!

    I'm not at that process yet but I agree with Casey that if the form is asking for it, there must be a reason. And the place you're getting married is your ceremony venue.

    I think the intended place is where the ceremony is taking place I.e., church. I would definitely put your parents middle names down (if they have one) if the form requests it.

    I haven’t gotten to this part yet. But the way I look at any official government documents. If it’s on the application there’s a reason for it. I would say put it down.
    As for the intended place of marriage. Could this be the name of the church, venue, or where ever your ceremony is. And then the city it’s in?
