Marriage licence Ontario - Maiden name required

Does anyone know if you are required to put your mothers maiden name on the marriage licence in Ontario? When I filled mine out it asked for Last name but did specify maiden name. I ended up just putting her married name. Does anyone know if I should re do it before going to the court house? I also put my MIL married name as well.
